Circumstances and some signs before gout attacks occur:
Clinical symptoms of gout are divided into two types of acute and chronic gout:
- Acute Gout:
Acute gout attacks are characterized by joint inflammation levels,
sudden onset, severe pain, accompanied by swelling, heat, redness in
joints hurt, the pain gradually increased and peaked pain usually after 8
to 12 hours, usually attack onset and increased pain at night, pain,
loss of sleep, sometimes 38 to 38.5 degrees fever, chills possible. Gout usually attacks initially affect only one joint, joint common location is the big toe joint goals (50%), knee, ankle. Matching wrist, finger table, elbow joints are common in acute attacks at a later stage. In this stage testing is generally elevated blood uric acid. However, there are nearly 40% of cases of normal levels of uric acid in gout attacks. Gout attacks are usually decreases after a few days, but may last for several weeks.
Gout attacks usually occur spontaneously or in many cases occur after a
meal with too much protein contains purines as human organ (heart pig,
duck blood, liver, kidney, brain, stomach, tongue ... ), red meat (beef
dogs, goats, calves ...), seafood (shrimp, crab, fish fat ..), pea nuts,
dried mushrooms, chocolate or alcohol .. excessive. Or after a treatment period lasted some diuretics, aspirin, anti-TB drugs ...
There are some warning symptoms gout attacks:
Headache, excited states, fatigue, digestive disorders, heartburn,
abdominal pain, strip, especially in places like symptoms: difficulty
moving the lower extremities, well vein, numbness toe ... This is the
best time to prophylaxis, not for acute gout onset (the patient should
consult your doctor promptly during this period).
For chronic gout:
If patients do not adhere to the prevention and treatment well, after
about 10 to 20 years with acute gout stage, progression of chronic gout,
grain bowl appearance fees (due to accumulation of sodium urate
crystals of salt to deposition of connective tissue) in many places,
such as ear, edge joints hurt .., chronic joint disease also cause
dangerous complications narrow slit joints, bone, kidney stones, chronic
renal failure . From here it can do for patients with lifelong disability and death.
The definitive diagnosis of gout:
Currently in Vietnam based on the diagnostic criteria of Bennett and
Wood 1968 is widely used to diagnose gout because it is simple, easy to
apply:
a - Or find urate crystals in synovial fluid or in the non-grain bowl.
b- Or at least two of the following four factors:
- A history of or current at least two phases of a sore joints with
nature sudden onset, severe pain, swelling, and completely out within
two weeks.
- A history of or current has a round table swollen big toe joint pain with the above properties.
- There are non-grain bowl.
- Responding well to treatment with colchicine (reduce inflammation, relieve pain within 48 hours) in history or current.
Diagnosis is confirmed to have a standard or at least two elements of the standard b.

As the doctor said it was gout cause dangerous complications for the
patient, even if not detected and treated in time will lead to death. So, when it was gout patients to be treated like especially about diet, exercise how to repel gout?
Treatment of gout:
- Treatment of acute gout attacks:
The consequences of acute gout is caused arthritis, therefore we have
used the drug group mostly anti-inflammatory, analgesic, such as:
+ Group nonsteroidal anti-inflammatory drugs: Drugs used is: Diclofenac
75 mg tube, deep intramuscular injection tube 1-2 / day, 2-3 days after
injection drug use to drink (like Meloxicam 7.5 mg, 20 mg piroxicam ...
) or Felden 20 mg intramuscular injection tube tube 1-2 / day x 3 days.
Colchicine +: This is the first drug of choice for treatment of acute
gout attacks, due to selective anti-inflammatory effects of this drug
that are still popular for years, the drug to be administered within the
first 12-36 hours of the attack gouty, the dose should consult a
specialist due to side effects of the drug can cause diarrhea, nausea,
and at risk of liver failure, kidney failure, bone marrow failure ...
+ Group Corticosteroids: Less use of the drug due to side effects,
however, can be used in some special cases: In patients with
inflammation of multiple joints at the same time, do not respond to
colchicine and anti-inflammatory drugs do not use steroids or some
preparations injected joints are inflamed.
+ Some other analgesics such as paracetamol (Efferalgan,
Efferalgan-codeine ...), patients can use alkaline salt 5g 1-2 nabica
pack pack / day mixed drinks or mineral water can be used to make
alkaline water alkaline Elementary, avoid urinary stones.
Prophylactic treatment of acute gout attacks:
It is the goal of reducing uric acid levels, limiting the deposition of
urate in the tissues and organs, preventing formation of chronic gout:
- Diet activities:
For patients with overweight and obesity should apply energy loss diet, fat loss.
Reduced protein diet (eat meat no more than 150g / day), especially
need to eat foods rich in purines such as animal viscera (heart pig
blood pudding, liver, kidney, brain, stomach, tongue) ... red meats and
seafood.
Abstinence from alcohol and stimulants such as pepper, coffee, pepper.
Drink plenty of fluids (water, juice, milk), especially with alkaline
mineral water (mineral water .. Quang Hanh) to enhance the excretion of
uric acid in the urine. Eat porridge, soup ...
Eat more vegetables, fruits like lettuce, carrots, cabbage, cucumbers,
melons, potatoes, tomatoes, bamboo shoots, vegetables artichoke. Can eat eggs, milk and white cheese preparations fermentation ...
Moderate activity, work gently, avoid cold, avoid tired both mentally and physically (excessive labor, injury ...). It should exercise appropriate and ongoing maintenance such as walking, swimming, badminton, table tennis, cycling ...
- Avoid taking some medications can increase blood uric acid as stated above.
- Drug prevention gout attacks: colchicine, allopurinol, thiopurinol, Benemid, uricozym ...
